Authorization for Repairs and Directions to Pay

I hereby authorize Goodfellas Collision Center to prepare an estimate for repair costs, if necessary dismantle parts of my vehicle, to do so. I understand that this is an authorization to repair and for evaluating damages for or the insurance company covering the cost of repairs. This document serves as direction to pay Goodfellas Collision Center directly for any and all repairs.

Goodfellas Collision Center will not be held responsible for items left in vehicle either towed or driven in for estimating or repairing. Any labor involved in dismantling or estimating must be paid before the vehicle is moved from Goodfellas Collision Center’s premise. Vehicles towed or driven in, deemed total loss, or moved to another location for any reason by customer or insurance company may be subject to admin fee, storage fee, or estimate fees. Any labor, towing, or lift inspection fees must be paid before the vehicle leaves Goodfellas Collision Center.
Outside Storage $45 - Inside Storage $55 Per Day.
